Your Initial Enrollment Period runs seven months: the three months before the month you turn 65, that month, and the three months after.

The California Birthday Rule

If you already hold a Medigap policy, California gives you sixty days from your birthday each year to move to a different policy with equal or fewer benefits, with no health questions asked.

Most states do not have this. It is worth marking on a calendar every year, not just the year you first enroll.

Already on Medicare, reviewing each year

Annual Enrollment runs October 15 to December 7, and any change takes effect January 1. Read your Annual Notice of Change each autumn before that window opens.

A plan's costs and network can change year to year, so a short review each autumn is how people avoid ending up on the wrong plan by default.

65 or older, not yet enrolled

If you have had coverage through your own or a spouse's current employer, you generally get an eight-month Special Enrollment Period once that job or coverage ends, whichever happens first.

COBRA does not count as employer coverage for this. The clock starts when the job ends, not when COBRA ends.

Before you compare anything, check two things first

Your doctors

Whether your specific doctors and hospitals are in a plan's network matters more than any other single detail. Check this before looking at anything else.

Your prescriptions

Check whether each plan's drug list covers what you actually take, and confirm it every year, since a drug list can change from one year to the next.

The San Diego County Aging & Independence Services program offers free, unbiased Medicare counseling through California's SHIP program (Health Insurance Counseling & Advocacy Program, HICAP).
